Thanks for that post. IT explains a couple of things I have been trying to understand as to how it works. I will try the all the changes you have suggested and let you know how I go when I get back from my holidays this week.

If have now added a one to one nat for the address and web browsing appears to be working.

I read the posts and then removed and recreated the connections and after checking all the passwords were the same, it worked.

Have you configured the ISP connection port as a DHCP client? My ISP connects to my CISCO router and assigns it an address. It also pushes the default route to it. How about setting up VPN's from those houses to your router then allowing the packets to access the PBX? All other solutions I can think of will require the networks outside your network access to pass their IP addresses. Not a good security feature.
