It's not count any packet.

Mikrotik is a Basic caching server that is caching only http web site & which is cacheable.
If you want to cache as your choice or more customization so you have to need squid.

it is possible by Limiting Download File Extensions.
Browsing is fine, but download is your choice.

I think Mikrotik Uerman is Good, But it's a Basic Radius Server, it's not for more customization.
You have to use another Radius Server for this type of (OTP) Service.
Like as Radius Manager (www.dmasoftlab.com), (https://www.open.com.au/radiator),
