While I haven’t confirmed it, I presume it means that with yes it will add and use the DNS that the L2TP server pushes to the client, along with any other DNS already configured on /ip dns, while with exclusive, it will exclusively use the DNS that the L2TP server pushes, ignoring any other DNS servers configured.
