RB1100AH2 running 6.15. I have duplicate mac addresses under IP > hotspot > host. Some mac appears as much as 10 times with different IP address.
How do I prevent this from happening?
Under hotspot server, I cannot set Address per Mac. That field is greyed out.
I also notice duplicate ARP entries for the same MAC with different IP address.

You have a Device using Proxy-arp.
Or someone install one “repeater” and use it to surf on your network…
This is a hotspot network. The radios are setup in WDS mode.
The hotspot interface on the mikrotik is set to ARP enabled.

and must remain on that settings.
If WDS and other thing works as expected, you not see any duplicate mac,
until someone not link one repeater (wds repeater, universal repeater, call it as you want)…
and you can see same mac with many ip
given the mac address - it might be a HTC Device somehow using Proxy-arp. Maybe as a “repeater”. the d8:e4:e0 is “nothing”- but d8:99:c4 is HTC. the d8:e4:e0 might be generated by this Device.
