Notice red logs…user-manager is not able to write logs to the SD.
Check the SD is write enabled.
I’d format the SD inside routerOS.
I wouldn’t use FAT32. Apart from being a crappy FS, this SD is going to be used exclusively by the router, so what’s the point?
Additionally, as per Dude server install Wiki entry:
If added disk by default is formatted with fat32 file-system then we recommend to re-format it to ext3 file-system.

IO errors generally means bad news.
Try a different SD card. Brand new, is not a guarantee that it’s not broken. It looks to me, like the SD card is faulty, or there’s perhaps some incompatibility between the MT and the SD card.
This thing is really complicated.
