I have RB493. I’ve upgraded OS to 4.16 from 4.11. Everything looks fine, but I can’t install correctly User Manager. When I type in browser http://ip/userman I’m getting message: 404 error. in Winbox in tool section I can’t see User Manager position. In log i have some errors about creating lines in database. Has somebody similar problem?? Please help:)
You HAVE substituted http://ip/userman with your real ip and not just type in http://ip/userman? E.g. http://192.168.88.1/userman or whatever your router interface IP is.
User Manager does not show in Winbox. This is normal. It is accessed only from web page interface and CLI.

May you check the free space on the disk? btw have you created to rebuild the DB?
Try to do this
/tool user-manager database rebuild
Or as reported by Ekkas (http://forum.mikrotik.com/t/rb450-disk-full-user-manager-logs-and-session/22827/1) , try before to delete also log entries (just if you see a disk space problem) :
/tool user-manager log remove
and then
/tool user-manager log remove
