I have a upgrade issue with just one cpe which originally had 3.31 and I transferred the 4.13 files and rebooted but did not come back on the network, lost wireless connectivity, had no problems using the same files for the rest of the clients and their upgrade was successful, so I thought maybe the ftp client did not transfer the files fully and I had not checked this, so no choice but call out to customer and login and noticed wireless package had not been installed , i then installed it and got the cpe operational on the network,
But then I tried to upgrade from 4.13 to 4.16 but this time I checked that all files were transferred and like before the 4.16 had been successful with other client cpe’s, so I rebooted and once again it did not come back and had to travel out to customer again, below is screen shot of wireless package which not installed on I rebooted the router (411+UB5 radio card) again which installed wireless but the wireless interface was disabled like when radio cards are installed.
Of course I am going to swap out this cpe for the customer in the New Year but I am curious why this is happening just to this cpe. 
install only those packages u need, and dont install nv2 pkg.
