Hi all,
Occasionally when I reboot this device, it doesn't come back up. The User and Fault lights stay on and nothing appears on the console. Is this a known issue at all, or do I likely have some faulty hardware?

Same issue here, but it happens at each soft reboot.
If I remove the PSU and wait around 5 minutes then plug it back in, then the switch starts well.
Happens wether I am using RouterOS or SwOS. It just does not boot, not even "bios".
I do not have the same PSU
Are you the one from the Reddit post? Mine was shipped to Poland for RMA and I don't have it back even now. This global semiconductor shortage is a nightmare.I just got 2 of these today from Streakwave. 1 of them is fine the other is doing exactly as the OP describes.
I can’t seem to find a solution other than to send it back to Streakwave for a refund since they don’t have any to exchange. Is there another option?

/system health detect-fans solved the problem!
Faulty unit shows four fans in system health.
After detect-fan, I can see just three fans.
