offering DHCP lease without success

I’ve run into an issue recently with my 3011 offering IP leases for certain devices without success. The exact error being “offering lease for without success.” This only started happening in the last few days without any updates having occurred and seems limited to a router and a few OS X devices. After pouring through these forums, I found that this error existed after firmware 6.38 but that it was later resolved. I’m using 6.41.3 currently (it was 6.41 but I upgraded to see if that would resolve the problem… no luck unfortunately). I’ve tried altering DHCP and RSTP, changing ports, and disabling/enabling DHCP as these were fixes others noted for earlier firmware. If anyone has any insights or suggestions, I’d love to hear them. I just find it a bit odd that I’m only seeing around 4 devices having issue while another 100 or so are connected fine.

As far as the network goes, this is a condominium guest network. I’m load balancing two different providers. The 3011 acts as the DHCP/DNS server and firewall for the network. Due to restrictions provided to me, that’s pretty much all that exists. The network is wide open and guests/owners are able to stick their own routers into their rooms without consulting me first (I’ve had many problems with people configuring their own stuff incorrectly and broadcasting DHCP back across the network but I’ve confirmed this is not the case here). Cabling doesn’t seem to be the issue either as the problem devices are throughout the 8 stories. No VLANS or anything that way. This network is about as basic as it gets: Mikrotik 3011 in the MDF with unmanaged switches on each floor and then 2 - 4 AP’s per floor. I’ve at least almost got them to OK’ing SWOS switches throughout the building and then creating segmentation to protect guests and minimize damage from misconfigured systems but I need this issue sorted first.
